THE 2008 GRAVITY PARK USA ANNUAL RIDERS RELEASE AGREEMENTS

The 2008 Gravity Park USA Annual Riders Release Signed Agreement allows a rider to participate in all of the Gravity Park USA track events including practice, open riding and races during the 2008 track season for the one year period.

THE EXCLUSIVE REGISTRATION CARD

New this year will be an agreement registration card that must be presented at the registration booth each time that you enter the track to participate in any event, including practice, open riding and racing events.  The card will be "swiped" through a registration processor each time that you are at the track.  There will be a fee charged for each card that you lose or do not have with you when you want to participate in any event at the track.  The processor is the first Race Track card processor in the State of Wisconsin.

INSTRUCTIONS FOR ADULTS AND MINORS

1. Adults, 18 years and older, please complete and sign the Gravity Park USA Annual Adult Release and Waiver of Liability Assumption of Risk Indemnity Agreement Membership Card Application Form. 

2. Minors, less than 18 years old, must complete and sign the Gravity Park USA 2008 Annual Minor Release and Waiver of Liability Assumption of Risk Indemnity Agreement.  Membership Card Application Form and the Gravity Park USA 2008 Minor's Assumption of Risk Acknowledgment Agreement Form.   Make sure to do BOTH FORMS!

3. The Adult agreement and the Minor Agreement must be notarized to be accepted by Gravity Park USA.

4. Mail the completed and notarized forms to:
