THE 2012 GRAVITY PARK USA ANNUAL RIDERS RELEASE AGREEMENTS

The 2012 Gravity Park USA Annual Riders Release Signed Agreement allows a rider to participate in all of the Gravity Park USA track events including practice, open riding and races during the 2012 track season for the one year period.

THE EXCLUSIVE REGISTRATION CARD

In 2012 you will be issued an  agreement registration card that must be presented at the registration booth each time that you enter the track to participate in any event, including practice, open riding and racing events.  The card will be "swiped" through a registration processor each time that you are at the track.  There will be a fee charged for each card that you lose or do not have with you when you want to participate in any event at the track.  The processor is the first Race Track card processor in the State of Wisconsin.

INSTRUCTIONS FOR ADULTS AND MINORS

All Waivers MUST be printed on a colored printer!

1. Annual Release, please complete and sign the Gravity Park USA Annual Release and Waiver of Liability Assumption of Risk Indemnity Agreement Membership Card Application Form. 

All Minors ( Riders and Spectators) entering the pit area must complete the minor waiver formS. If you are not accompanied by a parent, the waiver MUST be Notarized.

2. Minors, less than 18 years old, must complete and sign the Gravity Park USA 2011 Annual Minor Release and Waiver of Liability Assumption of Risk Indemnity Agreement  Parental Consent Waiver and the Gravity Park USA 2010 Minor's Assumption of Risk Acknowledgment Agreement Form.   Make sure to do BOTH FORMS!

4. Do not mail the waivers!! All waivers must be brought to the track with you.
